• Opt In Form for Overdraft Advance

Print out, sign, and turn in this form to enroll in Overdraft Advance to allow your personal checking account overdraw up to $350 with use of your debit card and ATM withdrawals.

• Change of Address Form

It’s very important to complete a Change of Address Form if your mailing address or contact information changes. As a financial institution, we have ‘Return Service Requested’ printed on the statement envelopes. It is up to you to change your address and phone numbers with us. We need you to contact us and fill out and sign a Change of Address Form. Please remember that mail from a financial institution is not the same as your magazine subscriptions and other mail you receive. Coloramo mail will not be forwarded if you winter somewhere else or move.

                          Get a Change of Address Form from one of our branches
                          Print one from this link

Have the primary member sign it and fax it back to 970-241-9726, drop it off in one of our night drops or bring it to any of our branches.
